Medical Records, Releases and Letters
To request medical records, complete a Release of Information (ROI), or request a letter or other documentation, please contact Alexis Lopez, DNP, PMHNP-BC directly through your patient portal or by contacting Shaka Mental Wellness.
Alexis will provide you with the appropriate request form to complete. Once the completed form and any required information are received, please allow approximately 7–10 business days for processing.
Some forms, letters, or other administrative requests may require additional time from the provider and may be subject to a fee. You will be notified of any applicable fee before the request is completed.
Crisis & Emergency Care
Shaka Mental Wellness is an outpatient psychiatric practice and does not provide emergency or crisis services. Messages, portal requests, emails, and voicemails are not continuously monitored and should not be used for urgent or emergency situations.
If you are experiencing a medical or psychiatric emergency, are in immediate danger, or believe you may harm yourself or someone else, call 911 or go to the nearest emergency department.
